Accident Summary:

THE AIRCRAFT WAS FLYING A RESUPPLY MISSION FOR THE CAV REG ROK TIGER DIVISION, 1ST BN, BR 689425 ELEVATION 20 METERS. ABOUT 1600 HOURS THEY LOADED ABOUT 1200 POUNDS OF WATER AND TWO KOREAN PACKS WHICH WAS TO BE TAKEN TO BR 631419 ELEVATION 200 METERS. A HOVER CHECK WAS MADE AND THE AIRCRAFT PULLED 39 POUNDS OF TORQUE WITH 6600 RPM. THEY DEPARTED 1ST BN TO THE EAST. THE WINDS WERE APPROXIMATELY 13007. THEY PROCEEDED TO THE LZ WHICH WAS ABOUT A FIVE MINUTE FLIGHT. SMOKE WAS POPPED UPON THEIR ARRIVAL AND THE APPROACH WAS SET UP TO THE SOUTWEST. ^1LT DAVIS^ WAS AT THE CONTROLS AT THIS TIME. DURING THE APPROACH ^LT DAVIS^ STATED THE WINDS SEEMED TO CHANGE DIRECTIONS THE APPROACH WAS TERMINATED AT ABOUT A 12 FOOT HOVER DUE TO THE TREES IN THE LZ. AT THIS TIME THE AIRCRAFT STARTED BLEEDING OFF RPM AND TURNING TO THE RIGHT. ^WO KANAUKA^ TOOK THE CONTROLS AND THE RPM STABILIZED AT 5800. WITH FULL LEFT PEDAL IN, THE AIRCRAFT CONTINUED A 90 DEGREE TURN TO THE RIGHT. ^WO KANAUKA^ BANKED THE AIRCRAFT TO THE RIGHT ATTEMPTING TO FLY DOWN THE HILL. THE AIRCRAFT COMPLETED A 360 DEGREE TURN AND SETTLED INTO THE TREES, EVENTUALLY COMING TO REST ON ITS RIGHT. ^WO KANAUKA^ PUT OUT A MAYDAY CALL, SHUT DOWN THE AIRCRAFT AND EVACUATED THE AIRCRAFT. THERE WERE NO INJURIES TO THE CREW OR THE PASSENGERS. THE ENGINE, A T53L13A, SERIAL NUMBER ^LE15140A^, HAD 308 HOURS SINCE LAST OVERHAUL WITH A TOTAL OF 1069:30 HOURS SINCE NEW. INVESTIGATION OF THE HISTORICAL RECORDS DID NOT DISCLOSE ANY CONTRIBUTING FACTORS TO THE ACCIDENT.
