Accident Summary:
AT 0745 HOURS ON 5 DEC 70, AIRCRAFT 67-17417 DEPARTED FROM ECHO ONE, LANE ARMY HELIPORT WITH A CREW CONSISTING OF AIRCRAFT COMMANDER CW2 ROBERT R. WEAVER, 519-54-0272, PILOT 1LT FRANK T. HAUSMAN, 497-48-4312, CREW CHIEF SP4 GARY W. FELDT, 570-76-7993, AND GUNNER SP4 ANDRADA, 567-64-4194, FOR A FLIGHT TO QUI NHON ARMY AIR FIELD WITH THE INTENT OF GETTING 1LT HAUSMAN HIS 90 DAY PROFICIENCY RIDE. ON TAKE-OFF, THE LEFT FUEL BOOST PUMP LIGHT BEGAN TO COME ON INTERMITTENTLY. THE BOOST PUMP LIGHT WAS NOTED AND FLIGHT CONTINUED UNTIL THE AIRCRAFT REACHED AN ALTITUDE OF 200 TO 250 FEET AGL AND AN AIRSPEED OF 70 KNOTS. AT THIS POINT, THE PILOT 1LT HAUSMAN, 497-48-4312, BEGAN A RIGHT TURN OUT OF TRAFFIC AND HEADED TOWARD QUI NHON. AN ENGINE SURGE WAS HEARD AND THE AIRCRAFT LOST POWER AND YAWED VIOLENTLY TO THE LEFT. AT THIS POINT AIRCRAFT COMMANDER CW2 WEAVER, 519-54-0272 TOOK THE CONTROLS, HEADED THE AIRCRAFT INTO THE WIND AND LOWERED PITCH. THE LOW RPM AUDIO AND LIGHT CAME ON, AS DID THE RIGHT FUEL BOOST PUMP. THE AIRCRAFT COMMANDER PUT OUT THE MAYDAY CALL AND THE PILOT LOCKED THE SHOULDER HARNESSES. WHILE DESCENDING, THE AIRCRAFT YAWED TO THE LEFT AND TO THE RIGHT AT LEAST TWICE MORE. AT APPROXIMATELY 100 FEET AGL, THE AIRCRAFT COMMANDER BEGAN AN AUTOROTATIVE FLARE TO SLOW THE AIRCRAFT. CW2 WEAVER PULLED THE INITIAL PITCH TO LEVEL THE AIRCRAFT AT AN ESTIMATED 10 TO 15 FEET AGL, AND ATTEMPTED TO CUSHION THE AIRCRAFT TO THE TOP OF THE SUGAR CANE. AFTER USING ALL AVAILABLE PITCH, THE AIRCRAFT FELL THROUGH THE CANE, RESULTING IN A HARD LANDING. THE AIRCRAFT HEADING WAS APPROXIMATELY 040 DEGREES. THE CREW OF 67-17417 WAS IMMEDIATELY PICKED UP AND THE AIRCRAFT WAS RECOVERED TO LANE ARMY HELIPORT, AT WHICH TIME THE ACCIDENT INVESTIGATION BOARD WAS ABLE TO VIEW THE AIRCRAFT.\\
