Accident Summary:
AT 1030 HRS., 8 MARCH 1970, WO1 HILL, THE PILOT WHO WAS AT THE CONTROLS, AND WO1 WAGNER WERE FLYING ON A COMBAT ASSAULT FOR THE 22ND ARVN DIVISION. THEY WERE CHALK THREE IN A FORMATION OF FOUR SHIPS. PRIOR TO LANDING AT THE LZ THEY SPLIT INTO TWO FORMATIONS OF TWO SHIPS EACH. THEY THEN BECAME THE LEAD SHIP OF THE SECOND FLIGHT WITH CHALK FOUR AS TRAIL. THE FIRST FLIGHT HAD JUST COMPLETED THEIR FOURTH SHORTIE INTO THE SAME LZ AND THE SECOND FLIGHT WAS ON THEIR FOURTH. THE LZ WAS ON A RIDGE LINE THAT SLOPED FROM NE TO SW. THE WIND WAS LIGHT AND VARIABLE BUT GENERALLY OUT OF THE EAST. THE APPROACH WAS MADE ON AN APPROXIMATE HEADING OF 150 DEGREES. THEY MADE THREE SUCCESSFUL LANDINGS WITH THE SAME NUMBER OF PAX. PRIOR TO THE ACCIDENT. ON THE FIRST THEY HAD 1200 POUNDS OF FUEL ON BOARD AND HAD NO TROUBLE TERMINATING AT A ONE FOOT HOVER IN THE LZ. ON THE APPROACH THE ACCIDENT OCCURED THEY WERE DOWN TO 500 POUNDS OF FUEL. BOTH PILOTS STATE THAT THE APPROACH WAS NORMAL AND NOT EXCESSIVELY FAST. THEY ENCOUNTERED NO PROBLEMS UNTIL THEY ATTEMPTED TO ERMINATE. THIRTY METERS PRIOR TO TOUCHDOWN WO1 WAGNER TOLD HIS CO-PILOT TO PULL IN MORE POWER. THE CO-PILOT STATES THAT HE FELT THE SHIP WAS HANGING IN THE AIR POSSIBLY DUE TO AN UPDRAFT OFF THE RIDGELINE, AND THAT HE REDUCED POWER WHEN THE CO-PILOT DID NOT RESPOND TO THE AC'S INSTRUCTIONS. THE AC STATES "I PUT MY HAND ON THE COLLECTIVE AND PULLED IT IN FAST ENOUGH TO STOP ALL OUR AIRSPEED AND RATE OF DECENT." THE AC MADE HIS LAST CHECK OF ALL INSTRUMENTS AT THIS TIME AND EVERYTHING LOOKED NORMAL. THERE WAS NOT ENOUGH POWER TO STOP THE DECENT AND THE LEFT SKID STRUCK THE UPSLOPE SIDE OF THE RIDGE BOUNCING THE SHIP BACK IN THE AIR AND RAPIDLY ROLL IT TO THE RIGHT. THE PILOT WAS UNABLE TO CORRECT THE ROLL TO THE RIGHT. THE PILOT WAS UNABLE TO CORRECT THE ROLL TO THE RIGHT. THE MAIN ROTOR BLADE STRUCK THE GROUND AND THE AIRCRAFT CONTINUED TO ROLL RIGHT COMING TO REST ON ITS LEFT SIDE.
