Accident Summary:

AT APPROXIMATELY ^1045 HOURS ON 14 JANUARY 1969 WO1 VICK AND WO1 SHARRER^ WERE FLYING AT A DESIGNATED TRAINING AREA, 1 MILE SOUTH OF ^LANE ARMY HELIPORT^. ^WO VICK,^ AN INSTRUCTOR, WAS GIVING ^WO SHARRER^ A REQUIRED 90 DAY CHECK RIDE IN A UH-1B, AIRCRAFT ^#64-13921^. AT THE TIME OF THE ACCIDENT, ^WO VICK^ WAS DEMONSTRATING A LOW LEVEL AUTOROTATION AND AT APPROXIMATELY 100 FEET AGL ON A HEADING OF 080 DEGREES. THE AUTOROTATION WAS INITIATED NORMALLY. JUST AFTER ENTERING AUTOROTATION ^WO VICK^ DETERMINED THAT IF HE CONTINUED THE AUTOROTATION WITH THE EXISTING AMOUNT OF FLARE HE WOULD HAVE GROUND RUN WHEN HE TOUCHED DOWN. AT APPROXIMATELY 50 FEET AGL ^WO VICK^ INCREASED HIS FLARE AND THEN BEGAN THE APPLICATION OF PITCH. FIFTY FEET IS THE ALTITUDE ESTIMATED BY BOTH ^WO VICK AND WO SHARRER,^ HOWEVER, IT IS MY BELIEF THAT THEY WERE CONSIDERABLY LOWER THAN ESTIMATED. AT THIS POINT GROUND CONTACT WAS MADE BY THE TAIL STINGER, TAIL BOOM AND ONE TAIL ROTOR BLADE CAUSING THE BLADE GRIP TO CRACK AND THE BLADE TO COME OFF. THE OTHER BLADE CONTINUED ITS ROTATION AND STRUCK THE GROUND. AT THIS TIME THE AIRCRAFT BECAME AIRBORNE AGAIN, CLIMBED TO APPROXIMATELY 6 TO 10 FEET AND YAWED TO THE RIGHT. THE YAW WAS DUE TO THE LOSS OF THE TAIL ROTOR. ^WO VICK^ STATED THAT HE STOPPED THE YAW BY ADDING A SLIGHT LEFT CYCLIC CORRECTION AND TRIED TO LEVEL THE SKIDS. THE AIRCRAFT CONTINUED ON ITS ORIGINAL HEADING WITH THE NOSE YAWING RIGHT FOR APPROXIMATELY 140 FEET BEFORE IT MADE GROUND CONTACT. AT THIS TIME THE AIRCRAFT MADE CONTACT ON THE HEEL OF THE RIGHT SKID AND BOUNCED INTO THE LEFT SKID. THE LEFT SKID DUG INTO THE GROUND APPROXIMATELY 6 TO 8 INCHES BREAKING THE REAR CROSS TUBE. THE FORWARD MOMENTUM OF THE AIRCRAFT CAUSED IT TO ROLL TO ITS LEFT SIDE, BREAKING THE FRONT CROSS TUBE AND CAUSING ONE MAIN ROTOR BLADE TO STRIKE THE GROUND AND THE OTHER TO STRIKE THE RIGHT SIDE OF THE ENGINE COMPARTMENT. THE MAST THEN BROKE AND THE AIRCRAFT CAME TO REST ON ITS LEFT SIDE. THERE WAS NO FIRE.
