Accident Summary:
ON 16 AUG 70, ^WO1 ALEXANDER^ AND ^WO1 GAMBRILL^ TOOK OFF ON A COMBAT ASSAULT MISSION IN SUPPORT OF THE ^22ND ARVN DIVISION^. ^WO1 ALEXANDER^ WAS FLYING WING SHIP OF A LIGHT FIRE TEAM. BOTH SHIPS IN THE FLIGHT WERE ASSIGNED TO TEE ^129TH AVN CO^ (AH). ^WO1 ALEXANDER^ AND FLIGHT REPORTED TO LZ CRYSTAL THE CREW PERFORMED THEIR DAILY MISSIONS AFTER RECEIVING THEIR BRIEFING. ENROUTE TO LZ CRYSTAL THE CREW PERFORMED THEIR DAILY ENGINE RECORDING WITH THE FOLLOWING RECORDINGS: N1 - 91.5,, EGT - 550, OAT - 29C. PRIOR TO THE MISSION ^WO1 ALEXANDER^ REFUELED AT LZ ENGLISH. FUEL CONTAMINATION CHECKS WERE PERFORMED BEFORE AND AFTER RE-FUELING, BOTH CHECKS WERE NEGATIVE. FROM LZ ENGILISH THE FLIGHT PROCEEDED TO THE AREA OF OPERATION, APPROXIMATELY 8 MILES SOUTHWEST OF LZ TWO BITS. WO1 ALEXANDER AND WO1 ALEXANDER AND WO1 GAMBRILL PROVIDED GUN SUPPORT FOR A FEW MISSIONS AND WERE FLYING A LEFT-HAND WAGON WHELL PATTERN AWAITING THE RETURN OF THE TROOP SHIPS. AT THIS TIME AN ENGINE FAILURE OCCURRED. AT THE TIME OF THE FAILURE THEY WERE AT 2,500 FEET MSL AND APPROXIMATELY 1500 AGL. ^WO1 GAMBRILL^ WAS AT THE CONTROLS OF THE AIRCRAFT WHEN HE HEARD A LOUD METAL TO METAL NOISE FROM THE ENGINE COMPARTMENT. ^WO1 ALEXANDER^ TOOK CONTROL OF THE AIRCRAFT WHEN THE ENGINE FAILED AND ENTERED AUTOROTATION. HE PERFORMED 5 TURNS TO REGAIN ROTOR RPM. ONCE ESTABLISHED IN AN AUTOROTATIONAL GLIDE, ^WO1 ALEXANDER^ TRIED TWICE TO JETTISON THE EXTERNAL STORES, BY MEANS OF THE ELECTRICAL JETTISON, WITH NEGATIVE RESULTS. APPROXIMATELY 75 TO 100 FEET ABOVE THE GROUND ^WO1 ALEXANDER^ :ECELERATED TEE AIRCRAFT AND PULLED INITIAL PITCH AT 15 TO 20 FEET. INITIALLY ^WO1 ALEXANDER^ PICKED WHAT HE THOUGHT TO BE A LEVEL OPEN RICE PADDY FOR TOUCHDOWN. INSPECTION OF THE RICE PADDY SHOWED IT TO BE FULL OF WATER, DEEP MUD, AND SEVERAL DIKES, CONCEALED BY 3 TO 4 FEET OF GRASS. THE AIRCRAFT FIRST MADE CONTACT WITH A DIKE WHICH SHEARED OFF THE SKIDS AND ROCKET PODS. THE AIRCRAFT THEN SLID APPROXIMATELY 50 FEET AND HIT ANOTHER DIKE. THE AIRCRAFT CAME TO REST ON A DIKE LAYING HALF-WAY ON ITS RIGHT SIDE.
