Accident Summary:
AT APPROXIMATELY 0900 HOURS, 6 OCTOBER 19698 THE 129TH AC (AH) WAS NOTIFIED THAT THEY WERE TO SEND ONE GUN TEAM (LIGHT FIRE TEAM) TO THE LOCATION OF 1ST COMPANY 1ST REGIMENT TO PICK UP AN AERIAL OBSERVER. THE CREW WAS ALERTED AND THE LIGHT FIRE TEAM LAUNCHED AT APPROXIMATELY 0915 HOURS. WHEN APPROXIMATELY THREE MILES FROM THEIR DESTINATION, TRAVELING IN A NORTH EASTERLY DIRECTION, THE DOORGUNNER OF THE LEAD A/C FLOWN BY MR. MARTIN SAID HE HAD SEEN SEVERAL SUSPECTED ENEMY CARRYING M-16 RIFLES IN A RICE PADDY TO THEIR EAST. MR. MARTIN NOTIFIED HIS TRAIL SHIP THAT HE WAS GOING TO MAKE A TURN TO THE RIGHT AND CHECK OUT THE SITUATION. WHEN THE TURN WAS INITIATED THEIR AIRSPEED WAS 80 KNOTS AND THEIR ALTITUDE WAS BETWEEN 75 AND 100 FEET. THE A/C HAD 900 POUNDS OF FUEL AND A FULL COMPLIMENT OF AMMUNITION FOR THE XM21 GUN SYSTEM ON BOARD. THEIR RATE OF TURN WAS APPROXIMATELY 35+040 DEGREES OF BANK AND THE TURN WAS INITIATED AT 80 KNOTS AND 75-100 FEET. WHEN THE TURN WAS ABOUT 3/4 COMPLETED HEADING GENERALLY SOUTHEAST, THEIR A/C BEGAN TO SLIP THROUGH THE AIR AND SIMULTANEOUSLY BEGAN TO FALL. THE FIRST PILOT MR. FITTS, SAID THAT THE RPM NEVER DROPPED BELOW 6600, ALL INSTRUMENTS WERE IN THE GREEN, BUT THE VERTICAL SPEED INDICATOR INDICATED A 1500 FOOT PER MINUTE RATE OF DESCENT. THE AC, MR. MARTIN, TRIED TO REGAIN CONTROL BY APPLYING AFT CYCLIC AND LEVELING THE SKIDS, BUT THE A/C REMAINED OUT OF TRIM AND DID NOT RESPOND. AT THAT MOMENT THE A/C WAS OUT OF CONTROL, THUS STRIKING WITH THE LEFT SKID A RICE PADDY DIKE WITH FORWARD AIRSPEED AND A HIGH RATE OF DESCENT. AFTER INITIAL CONTACT WITH A SECOND DIKE AND A TREE. AFTER FINAL CONTACT WITH THE TREE, THE A/C CAME TO REST ON ITS LEFT SIDE. NO POST FIRE OCCURRED BUT MAJOR A/C DAMAGE AND SERIOUS CREW INJURIES WERE SUSTAINED. THERE WERE NO FATALITIES AND NO COLLATERAL INVESTIGATION WILL BE CONDUCTED.
