Accident Summary:

ON 1 JUNE 1970, WO1 ROBINSON AND WO1 ALVAREZ TOOK OFF ON A COMBAT ASSAULT MISSION IN SUPPORT OF THE 4TH INFANTRY DIVISION. THE FLIGHT CONSISTED OF TWO ARMED UH-1B AIRCRAFT, BOTH ASSIGNED TO THE 129TH AVIATION COMPANY (AH). WO1 ROBINSON'S AIRCRAFT WAS FLIGHT TEAM LEAD. AT APPROXIMATELY 1630, AFTER REFUELING, WO1 ROBINSON CALLED THE LOCAL PATHFINDER CONTROLLING AGENCY FOR DEPARTURE CLEARANCE. HE WAS CLEARED FOR A WEST DEPARTURE, RUNWAY HEADING 240 DEGREES. WO1 ROBINSON BACK TAXIED APPROXIMATELY 50 FEET IN ORDER TO PROVIDE MORE TAKE-OFF DISTANCE SINCE HE HAD EXPERIENCED SOME DIFFICULTY TAKING OFF EARLIER THAT DAY. HE COMPLETED HIS HOVER CHECK, NOTING ABOUT 95% N1 AND 6350 RPM, CALLED FOR AND RECEIVED TAKE-OFF CLEARANCE. JUST SHORT OF THE SOUTHWEST END OF THE RUNWAY, 645 FEET FROM THE TAKE-OFF POINT, THE AIRCRAFT REACHED AN ALTITUDE OF 6 TO 10 FEET, JUST CLEARING A ROLL CONCERTAINA WIRE AT THE END OF THE TAKE-OFF LANE. ALMOST IMMEDIATELY AFTER CLEARING THE WIRE, THE AIRCRAFT STARTED SETTLING. WO1 ROBINSON SAW THAT GROUND CONTACT WAS INEVITABLE AND PULLED IN COLLECTIVE PITCH IN AN ATTEMPT TO CUSHION THE GROUND IMPACT. FIRST GROUND CONTACT WAS MADE 180 FEET BEYOND THE END OF THE TAKE-OFF LANE, WITH THE LEFT SKID STRIKING FIRST, FOLLOWED ALMOST IMMEDIATELY BY MUCH LIGHTER GROUND CONTACT WITH THE RIGHT SKID. THE AIRCRAFT BOUNCED, DRAGGING CONCERTINA WIRE WITH IT AND CAME TO REST 30 FEET BEYOND THE FIRST POINT OF IMPACT. AT THE INITIAL POINT OF IMPACT THE AIRCRAFT HEADING WAS CRABBED APPROXIMATELY 5DEG RIGHT. AT THE SECOND AND FINAL IMPACT POINT, THE AIRCRAFT HEADING WAS PERPENDICULAR (335DEG) TO THE INITIAL TAKE-OFF HEADING (240DEG). VISUAL EVIDENCE AND WITNESSES' STATEMENTS VERIFY A FIRE INSIDE THE ENGINE COMPARTMENT AROUND THE DIFFUSER SECTION. THE FIRE EXTINGUISHED ITSELF IN APPROXIMATELY 30 SECONDS AFTER THE AIRCRAFT CAME TO REST.
